Whether in their own apartment, a group home or long-term care facility, we serve our clients in an environment that is comfortable and familiar to them. To better serve our clients, we maintain a small provider-to-client ratio so we can focus on providing intensive and individualized services to clients with the greatest needs. Our services are tailored specifically to our clients, building upon their strengths to develop skills, boost confidence, and increase independence.

Adapt Expands to Alton, Illinois
Now available to individuals in the Metro East area (Madison, St. Clair, and surrounding counties), Adapt has expanded services to Alton, Illinois.
This on-site program, located within a nursing home setting, provides specialized mental health services for adults diagnosed with Serious Mental Illness. Designed for individuals who require additional structure and support, the program helps residents develop the skills needed to increase independence and prepare for successful community living.
Services are grounded in a psychiatric rehabilitation model and delivered through a person-centered, strengths-based approach. An interdisciplinary team of Licensed Practitioners of the Healing Arts, Qualified Mental Health Professionals, and Mental Health Professionals provides coordinated care in a safe, trauma-informed environment. The program emphasizes positive, supportive approaches that promote stability, personal growth, and meaningful engagement.
Participation requires dual admission to both the nursing home and the Adapt program. Admission is based on an assessment of each individual’s strengths, needs, and the program’s ability to safely and effectively provide services within the nursing home setting.
